External lesson learned: Fatal accident DCV Balder (HMC)

On 17 October 2013 there was a fatal accident on board the Balder (DeepWater Construction Vessel), belonging to HMC.

The Permit to Work Controller (PTWC) was leaving the ballast tank.

While he was climbing the ladder the gas meter was hung around his neck. The gas meter got caught behind a supporting bar for the ladder which made the PTWC lose his balance and fall off backwards to the foot of the ladder.
