Lesson Learned - Dropped Scaffold Tube

A scaffold crew were constructing a system scaffold and were about 22m above the cellar deck of the platform. The first standard was topped out and one of the crew moved on to add the second standard.  This is connected via a spigot which is bolted on to the lower standard.

During the course of doing this, the second standard fell about 22m, travelling horizontally about 10m, coming to rest on the cellar deck, which has not been blocked off.

The incident was reported to supervisors on site, but was not reported to the overall Stork supervisor. Due to this, the onshore project management team were unaware of the incident until the following day.
