Lessons learned - employee falls off stepladder

An employee used a stepladder to cut a ¾” pipe with a torch at a height of 2.5m. Once the pipe was cut it bent more than expected and hit the stepladder, resulting in the employee losing their balance and falling on the floor. The IP used his right hand to break his fall, causing a fracture to his right wrist. He also hit his head on the concrete floor.
The employee was immediately brought to the hospital for medical treatment where surgery was needed due to the fracture of his right wrist.
