Lessons Learned - Reinstating Grating Fall

A rope access team consisting of two people were reinstating gratings following fabric maintenance works.

The injured party (IP) was using a rope access positioning system to protect themselves from a potential fall through the open deck.  As the IP was moving backwards along the beam, he was holding onto the adjacent handrail with the their left hand and the grating section was lifted using a lifting tool in the IP’s right hand. 

As the IP moved his left foot to progress along the beam, he caught it and lost his footing.  In doing so, the IP’s hand came away from the handrail and the IP fell 0.5 metres, colliding with the middle horizontal bar of the handrail, impacting the IP’s ribcage/torso area.
