Lessons Learned - Tripped on plywood sheet

An offshore worker was injured while walking on a level walkway when he tripped on raised ply-board sheeting and collided with an adjacent fixed scaffold access ladder.                        

The ply-board sheeting had been placed at this location by an unknown person / work party, who may possibly have had good intentions in protecting the underlying GRP grating or to mitigate the risk of potential dropped objects. However, due to wear and weather deterioration the ply-board had developed into a substandard condition because of the length of time in position.

The injured party failed to notice the condition of the ply-board or recognise this as a potential hazard and subsequently tripped over a raised edge, colliding with the fixed scaffold ladder resulting in an injury to his neck and shoulder. His injuries subsequently required medical treatment from the platform medic.
