Lessons Learned - WCC Non-Compliance

Two rope access technicians (RAT1) and (RAT3) were employed as part of a three person team to install electrical cables. This involved working at height. Prior to the work commencing a Work Control Certificate (WCC) was raised. The Performing Authority had conducted a toolbox talk and ensured that the task, hazards and controls were discussed and understood by the team.

However, it was observed that the RAT3 was completing the installation of tie-wraps at the work location at height without other work party personnel present or within visible or audible contact. The other team members had temporarily left the work area to collect materials (tie wraps) from an adjacent module. The tie wraps were required to finish the job. Although a detailed rescue plan document was available at the work site, there was nobody on the site at the time to implement it if required and no identified means of contacting them.
