Safety Alert - Dropped scaffolding fitting

A team of four scaffolders were working on a scaffold on an offshore installation. Being short of one fitting, they requested a new one be passed up.

The scaffolder that passed up the fitting took the decision to throw the single fitting up to the top of the scaffolding.

In doing so, the scaffolder misjudged his throw and the fitting struck a metal beam above the scaffold. This caused it to bounce back over a handrail, resulting in the fitting falling to sea.
