Lessons Learned - Near Miss Potential Fall

A qualified team of rope access technicians were conducting grounding work within a 30' elevated Rack.  Rope access was being utilized as the primary method of access and egress from the pre-determined work areas.  The scope required that two technicians be in close proximity to each other during the task.  One of the technicians noticed the other had removed his primary and secondary retention.  A stop work authority was issued.
