USA Lessons Learned: Intervention - Stop Work Authority

Rope access was being utilized as the primary means of access. During the ascent to the work area the rope access technician was observed to have only one point of contact. The rope access supervisor utilized his Stop Work Authority (SWA) which was not acknowledged by the technician, but corrective action was applied and the supervisor allowed work to continue.
