Safety alert - Hydraulic torquing tool
Work was ongoing to change out an orifice plate on a 14" flanged gas lift supply line. The team had experienced difficulty in loosening the nuts. After successfully removing a nut from the back of the flange, the square drive hydraulic torque wrench with a socket attachment was being used to tighten the remaining nut onto the flange in an attempt to remove the stud bolts. As the nut tightened and the stud bolt withdrew, the bolt forced the socket off the nut.
The hydraulic unit detached from the flange and struck the peak of the operator’s hard hat and he suffered a soft tissue injury to the right side of his forehead. The investigation into this incident is ongoing.
